Health Insurance Marketplace Notice
The Affordable Care Act (ACA) provides for the establishment of a Health Insurance Marketplace (also known as an Exchange), where individuals can purchase health insurance coverage.
A copy of the IRS Form 1095-B or Form 1095-C can be received by the contacting Laser Access LLC Human Resources department. Requests will be fulfilled within 30 days of receipt or by January 31st whichever is later.
Employees of Laser Access LLC may be eligible to purchase health insurance coverage through the Marketplace and may qualify for a premium tax credit that can reduce the cost of coverage, depending on household income and other eligibility factors.

If you choose to purchase health insurance coverage through the Marketplace instead of enrolling in coverage offered by Laser Access LLC, you may lose any employer contribution toward employer-sponsored health coverage.
